Fee Schedule
Self-Pay/Out-of-Network
The following fees apply to patients who do not have insurance coverage or whose services are not covered through their insurance plan.
| Initial Diagnostic Evaluation (1 hour) | $300 |
| Follow-up Med Management (30 min) | $150 |
| Follow-up Med Management (60 min) | $200 |
| Same-day Crisis appointment | $200 |
| Disability Paper work (minimum 5 visits | Starting at $100 |
| Letters (Including Pet Support) | $35 |
**Please Note: A fee of $75 for 30-minute appointments and $100 for 60-minute appointments will apply for missed appointments or late cancellations without at least 24 hours notice, regardless of insurance status.**
